Data controller
The controller of personal data is Aida Muweis, Specialist Psychology Practice, ul. Kapelanka 6A/116, 30-347 Kraków. Contact for data protection matters: kontakt@aidamuweis.pl.
Website hosting
The website is hosted on servers of Cloudflare, Inc. (USA). Each time a page is displayed, Cloudflare technically processes your IP address and basic browser information in order to deliver the page and protect it against attacks. The legal basis is my legitimate interest in running the site securely (Art. 6(1)(f) GDPR). Cloudflare transfers data to the USA under the EU-US Data Privacy Framework and standard contractual clauses.
Contact form
The data you provide in the form (name, e-mail address, subject, message and page language) is processed solely in order to reply, on the basis of your consent (Art. 6(1)(a) GDPR). Providing the data is voluntary, but without an e-mail address I cannot reply.
Messages are transmitted through the Formspree, Inc. (USA) service, which acts as a data processor and forwards them to my e-mail inbox. Data is transferred to the USA under standard contractual clauses approved by the European Commission. I keep the data for as long as needed to handle the correspondence and then delete it.
Booking visits
Visits are booked through the ZnanyLekarz service. Data processing in that service is governed by its own privacy policy.
Map
By default the site shows a static map image that does not connect to any external service. The interactive map loads only after you click the “Show interactive map” button – your browser then connects to servers of the OpenStreetMap Foundation (United Kingdom), which receive your IP address. The “Get directions” link opens Google Maps in a new tab; from then on Google's privacy policy applies.
Cookies and statistics
This site does not use cookies or tracking tools. Aggregate visit statistics may be collected with Cloudflare Web Analytics, which does not set cookies, does not build profiles and does not identify visitors.
Your rights
You have the right to access your data, to rectify, erase, restrict or port it, to object to processing based on legitimate interest, and to withdraw your consent at any time (without affecting the lawfulness of earlier processing). You also have the right to lodge a complaint with the President of the Personal Data Protection Office (UODO).
